To apply for a medical visa for Thailand, you will need to prepare and submit the following documents:
- Valid for at least six months.
- Include the last two passports.
- Provide a GD copy (translated into English and notarized) if a previous passport was lost or missing.
- Submit a marriage certificate if your spouse’s name is not on the passport.
- Include an affidavit if family member names differ (e.g., father, mother, spouse).
- Two recent passport-sized photos (3.5×4.5 cm) with a white background on matt paper.
- Six months of bank statements, with regular transactions (minimum five per month).
- Bank solvency letter with a minimum balance of BDT 60,000 for single applicants or BDT 1,20,000 for families.
- For business owners, submit an updated trade license (translated and notarized), company letterhead with active contact information, and a business card.
- For employees, provide a salary certificate, pay slip, or salary account statement, and a copy of your office ID card.
- For government employees, include a GO letter. If retired, attach a copy of retirement documents.
- For students, submit a valid school ID card with the school’s contact number and an official NOC.
- Submit recent medical reports or a doctor’s referral letter.
- Provide a round-trip air ticket or e-ticket copy.
- Submit the latest academic certificate if currently unemployed.
- Required if applying as a domestic worker.
- Provide two active phone numbers of the visa applicant.
- Include one guarantor’s phone number, full name, and address.
